Failure to provide vet records WILL DELAY the review process and lead to potential denial of pet relinquishment. The information above is the most current information available and answered to the best of my knowledge. I understand that by completing and submitting this form that I am relinquishing my animal to Hospice Hearts. I will pay the relinquishment fee of $50.00 or $100.00 (per animal, if incomplete or no vet records) by the date of which the animal goes into a foster home. Once accepted by Hospice Hearts, it is my responsibility to transport my animal to the designated foster home or animal coordinator.
