February Mindful Walk Registration

*New: Please help us by using one form for each participant. In other words, each participant should register separately. Here's why:

  • In compliance with current New York State Covid-19 regulations on non-essential gatherings, we are capping the total number of program participants (including leaders) at 10. One participant per form keeps our registration numbers accurate, and that helps keep everyone safe.
  • We need all participants to read and agree to the terms of our Participant Waiver and Release of Liability.
