The Westmoreland Symphony Orchestra has put in place preventative measures to reduce the spread of COVID-19. These measures are enacted upon guidance from federal, state, and local authorities, also to work in tandem to any facility the WSO may be utilizing as a performance space. By reading and receiving this memorandum, you voluntarily agree to hold harmless the Westmoreland Symphony Orchestra in terms of any illness, damage, loss, lawsuit, or expense (to any extent of harm) as a result of attending a performance with the Westmoreland Symphony Orchestra in regards to COVID-19 or related health or financial symptoms/conditions.

To Whom It May Concern: I give my child permission and consent to participate in publicity and media activities on behalf of the Westmoreland Symphony Orchestra Academy of Music organization or activities related to WSO organization events, rehearsals, concerts or publicity, including photographs, reproductions, audio/video recordings, recordings for television, radio or social media, webcasts, interviews, etc., and to exempt the WSO organization from obligation and/or remuneration for use of such material for publicity, marketing or other purposes. Image/Video Use Permission I further hereby grant permission for photographs/image files and/or video files of my son/daughter to be posted on the WSO organization web site, You Tube channel and on other electronic communication tools/social media, and to be posted on the web sites of foundations or agencies or included in annual reports, publications or publicity related to WSO organization activities, reports, programs, materials or events.
