PSAT Test Registration

I am executing this Waiver and Release of Liability and Indemnification (this "Release") as a condition to the enrollment of my child listed above as a participant ("Participant") in the Standardized Testing offered by Lucas Christian Academy ("LCA"). This Release covers all LCA Standardized Testing to be held at the LCA facilities at 505 West Lucas Road, Lucas Texas 75002. By checking the “I agree to the above waiver”, as the participant’ s legal guardian expressly understand, assume and consent to all the terms, conditions and risks set forth herein. 1. Participant will take part in LCA Standardized Testing to be held on October 13, 2018. 2. I give my consent for LCA to obtain or administer emergency healthcare to my child. By agreeing below, I declare that I have the understanding and capacity to communicate health care decisions for this student and that I am fully informed as to the contents of this document and understand the full import of this grant of powers to LCA. 3. I hereby Indemnify, release and discharge LCA and its owners, directors, officers, employees and agents from any liability, claims, losses, judgments, costs, or expenses arising directly or indirectly from Participant’s activities in the LCA Standardized Testing , including claims or damages resulting from death, personal injury, partial or permanent disability or property damage, medical or economic losses, including attorney’s fees, whether caused in whole or in part from any instruction or training hereunder and whether based upon the breach of any express or implied warranty, negligence or under any other legal theories. I further indemnify, release, and forever discharge LCA from any liability, claims, losses, costs or expenses arising directly or indirectly from Participant’s use of the Lucas Christian Academy facilities, equipment or participation in LCA University Days Summer Enrichment Program courses. 4. I acknowledge that this Release shall be binding upon me, my spouse, Participant’s other legal guardians, respective heirs, executors, administrators, and legal representatives. I represent and warrant to LCA that I am in fact duly qualified to act for and bind Participant and am authorized to do so. As used herein, the term “LCA” shall include its duly authorized directors, officers and employees and agents. This Release commences in effect for October 2018.
