ChadTough Defeat DIPG Foundation is grateful for your interest in fundraising on our behalf. Please fill out the following information a minimum of two weeks before your event.

Please provide a brief description of your event. If you'd like the event to appear in the Upcoming Events section of our website, this description should be exactly as you'd like it to appear on our website (150 words max).


(If yes, please refrain from contacting businesses that have already committed to supporting ChadTough Defeat DIPG. A current list can be obtained by emailing amy@chadtough.org.)
Event Agreement

If any significant details for my event change after submitting this form, I agree to notify amy@chadtough.org as soon as possible. I understand that the ChadTough Defeat DIPG Foundation reserves the right to direct me at any time to remove any references to the foundation from my event if the foundation determines the event to be inconsistent with its mission, vision, or values.

In holding a fundraising event to benefit the ChadTough Defeat DIPG Foundation, I agree to obtain all necessary insurance, licenses, and permits to conduct the event at my own cost. I will refrain from applying as the ChadTough Defeat DIPG Foundation and/or using the foundation's tax ID. I am aware that events involving raffles, bingo, alcohol, or other games of change may have special regulations and/or required permits.

I understand that event organizers are responsible for payment of all event expenses. If event expenses are greater than money raised, I agree that the event organizers will pay those expenses. I understand that the foundation cannot provide reimbursement of any expenses related to my event.

Please reference your event name when submitting. We ask that all donations be sent within 30 days after completion of the event.

Event participants, donors, and sponsors should be provided with accurate information about the use of funds in promotion of your event. The following are guidelines from the Better Business Bureau:

Money Going to ChadTough Defeat DIPG... Language to Use...
If all the money collected is going to ChadTough Defeat DIPG... All proceeds to benefit the ChadTough Defeat DIPG Foundation
If all the money minus your expenses for the event is going to ChadTough Defeat DIPG...

A portion of the proceeds to benefit the ChadTough Defeat DIPG Foundation

or

All net proceeds to benefit the ChadTough Defeat DIPG Foundation

If a specific dollar amount in the purchase of a product or service is going to ChadTough Defeat DIPG...

or

If a set donation amount is going to ChadTough Defeat DIPG...

You must state the dollar amount.

Example: For every pizza we sell on Thursday, $5 will go to the ChadTough Defeat DIPG Foundation.

or

Example: A donation of $1,000 will be made to the ChadTough Defeat DIPG Foundation.

If a percentage of the proceeds is going to ChadTough Defeat DIPG...

You must state the percentage of the proceeds that will benefit the foundation

Example: 30% of the proceeds (or net proceeds) to benefit the ChadTough Defeat DIPG Foundation.
