2020 Y-WE Creativity Seminar Enrollment Form

Thank you for your interest in our online Creativity Seminar program offering! During these times of uncertainty, we believe it is vital to stay connected as a community. To that end, we have adapted our curriculum to provide weekly programming online for youth in the wider Y-WE Community. 

Y-WE Creativity Seminars are a space to practice creative thinking & design using the arts and what we have available in our homes and hearts. This online program is hosted every Wednesday at 1:30 pm PST and curated by Y-WE Teaching Artist, Kamilla Kafiyeva.

This enrollment form is for young women*, ages 13-18, who wish to enroll in our online Creativity Seminars.

Please look over the form below and prepare to complete and submit it in one session. You WILL NOT be able to save the form mid-way through.
