Pet Information

Please Note: HSH will provide you with the medication that is best suited for your pets weight. We will be unable to weigh your pet at the clinic. For this reason, it is important that you know the exact weight of your pet in pounds before submitting this form.

Please Note: We are only able to sell medications that are weight appropriate for the pet that you describe in this registration. It is your responsibility to ensure that the information you provide to us is accurate, as we will distribute the weight appropriate medication based on the information you provide to us here. We are unable to provide medications that are not weight appropriate based on the information you provide in this registration (ex. a larger Nexgard pill to split between two animals). There are no exceptions to this rule. If your pets weight at the time of pick up does not visibly resemble the information provided in this form, HSH will be unable to distribute any medication and your purchase will be considered a donation to the shelter.  There are no refunds, exchanges, or reschedules available to those who do not complete this form with accurate information.

The Humane Society of Harlingen will be hosting a low-cost community clinic for dogs on Saturday, June 26th beginning at 10 AM with a hard stop at 1 PM. We will cease all vaccinations and medication distribution at 1 PM as that is when the veterinarian leaves the shelter. We recommend getting in line no later than 11 AM to ensure that you are seen before 1 PM. The earlier the better! You must pre-register and pre-pay online for this clinic. Pre-registration is limited to 300 dogs and 100 cats. Following pre-registration, you will be given a unique confirmation code. Please write down or take a photo of this code, as it is your proof of purchase. During this clinic, HSH will be providing discounted vaccinations for Canine Rabies, Distemper/Parvo and Bordetella. HSH will also be selling canine dewormer, Nexgard Canine Flea & Tick Prevention, and TriHeart Canine Heartworm Prevention. Dogs MUST BE present in order for medications to be distributed. Medications will not be distributed if the animal described in the registration form is not present.
