Winter Shelter Volunteer Information Form

Thank you for your interest in Volunteering for Winter Shelter of Forest Grove and Cornelius!!

If you indicate interest in the roles of Overnight Host or Laundry Host, you will be asked to to complete a Background Check Form. This will be processed by our partner United Church of Christ. Complete confidentiality is held by responsible parties, at Winter Shelter of Forest Grove and Cornelius. If you want to discuss this policy with our Director, there are some alternatives available on a case-by-case basis. We have a strong philosphey of meeting people where they are, while ensuring the saftey and well-being of our guests, volunteers, staff, and host spaces.

Media Release

By entering my name above I authorize Winter Shelter, and the media organizations that it authorizes, now and in the future, to photograph and sound record my activities, myself, and my surroundings while engaged in Winter Shelter related activities. I understand the media records may be used for informational, instructional, and promotional purposes for Winter Shelter only and will not be used to generate a profit or for any other commercial purposes. I understand the media may be used throughout Winter Shelter, the local community, the city, state, and with federal agencies for the diversity of their needs. I have not been compensated, nor will I seek compensation, for the media use. I release Winter Shelter from responsibility should a third party violate the terms of this release.

Liability Release

By entering my name above I recognize that as a participant at Winter Shelter located at the Forest Grove United Church of Christ, Emanuel Lutheran Church in Cornelius, or sponsored by Old Town Forest Grove Church, I hereby release, acquit and hold harmless those organizations, their employees, representative, other participants, and any and all other organizations, businesses, governmental entities whatsoever from and against all claims, demand and causes of action for any injury, damages or death related to my use or presence at, or for, any of those organizations, their facilities, properties and their provision of any services to me.

Confidentiality Agreement

Winter Shelter takes guest privacy seriously. All participants are responsible for maintaining and protecting the confidentiality of information as it relates to guests and Winter Shelter. Maintaining confidentiality will be in compliance with the law, will enhance trust between guests and Winter Shelter, and will respect the guest’s and Winter Shelter’s right to privacy. As a participant of Winter Shelter, I do hereby affirm, by entering my name above, that I will treat all Winter Shelter guests’ and organizational information as confidential. I understand that I may convey information concerning guests to shelter staff members, and/or other participants only as necessary and only for the proper provision of service to the guest and safety of guests and participants. I will not divulge any information regarding guests either directly or indirectly for any other purpose.
