Please enter your Contact Information and the title, size and medium of each painting you are donating. Look for examples below each entry box. If you are donating something other than a painting, please do the best you can with this form. You can use the "Title" or "Medium" boxes to discribe your item.
After you enter the information, look for the "Choose File" button. Click on the button and up-load the JPEG image of your donation. Do this for each donation. When you are finished click the "Submit" button at the bottom of the form.
NOTE: the name of your JPEG should be: First Name_Last Name_Title_Size_Medium
