To submit your digital proxy, please fill out the following form with your name, email and membership number if you have it. If you plan to attend the Annual Meeting and Holiday Party with any non-member guests, please indicate how many.
After you click submit, you will be directed to the payment screen. If you are not attending with any non-member guests or prefer to pay for them by check, you can close that screen without paying and we will still receive your completed form.
