Firewise USA Community
Homeowner or Renter Self-Assessment Survey

We are required to ask all residents in the greater Cement Hill neighborhood to provide information about the characteristics of their homes and the vegetation management within their home ignition zones.

Please submit your form by July 1, 2023.

To more easily respond to some of the questions,
answer as if it is July 1st.

This information, as percentages, will be compiled for inclusion in GCHNA's application to qualify again as Firewise Community.

Nothing in this document will be used for any other reason. Please be honest in your answers.

If you would like to print out a copy of the survey to send by mail instead, click here to download the printable form.

If you have any questions, contact Ellen Hagan, firewise@gchna.com.

Thank you!
