Description of your art related activity

All participating groups are encouraged to have an arts related activity. This can be a simple arts and craft project, a demonstration, or something related to the arts. Traditional categories within the arts include but are not limited to literature (including poetry, drama, story, and so on), the visual arts (painting, drawing, sculpture, etc.), the graphic arts (painting, drawing, design, and other forms expressed on flat surfaces), the plastic arts (sculpture, modeling), the decorative arts (enamelwork, furniture design, mosaic, etc.), the performing arts (theatre, dance, music), music (as composition), and architecture (often including interior design).

Waiver and Release

The organization/individual completing this registration agrees to hold ShoreLake Arts, City of Shoreline, City of Lake Forest Park, their agents, employees, and officials, while acting within the scope of their duties, harmless from all causes of action, demands and claims, including the cost of their defense, arising in favor of the organization, individual, or third parties on account of personal injuries, death or damage to property arising out of activities at the premises and in any way connected with the facilities or in any way resulting from the willful or negligent acts of omissions of the organization, its agents, employees or representatives.
