Registration Form: 2023 Empowering Youth Leadership Institute

Empowering Youth Leadership Institute is open to 9th-12th graders. It will be held at Camp Paxson in Seeley Lake, and will occur from Monday, July 24th at 3:00pm to Wednesday, July 26th at 12:00pm.

The goals of this 3-day leadership camp are to strengthen individual leadership skills & empower participants to create positive change within their community. After this experience, participants can co-lead empowerment workshops rooted in social justice in k-12 schools across Montana. The Empowering Youth Leadership Institute increases participants’ Social Emotional Learning Outcomes and gives them opportunities to develop healthy relationships with peers and adults. Plus, we have a lot of fun!

COVID-19 safety protocol

Voluntary participation

I acknowledge that my (my child’s) participation in EmpowerMT’s programs is voluntary and has inherent risks and dangers that cannot be eliminated. Prevention of harm

I understand that EmpowerMT employees and volunteers will try to prevent accidents. However, I fully understand that some activities I (my child) will participate in may involve inherent risks regardless of all feasible safety measures being taken by EmpowerMT.

Liability for personal property

I agree to accept responsibility for any loss, damage, or injury to my (or my child) that occurs during my (my child’s) participation in this program that is not the result of fraud, willful injury to a person or property or the willful or negligent violation of the law.

Transportation

For participants 18 and under

I give permission for my child to ride in buses, cars, and other vehicles that are hired, rented, or driven by EmpowerMT staff and volunteers.

Emergency care

In the event it becomes necessary for the EmpowerMT employees and/or volunteers to obtain emergency care for me (my child), EmpowerMT nor its employees and volunteers will assume financial liability for expenses incurred because of an accident, injury, illness and/or unforeseen circumstances. I authorize EmpowerMT employees and volunteers to obtain all necessary emergency medical care and authorize any licensed physician and/or medical personnel to render necessary emergency treatment to me (my child).

Photo & video release & consent

EmpowerMT may photograph and record me (my child). I grant EmpowerMT permission to use and publish images and videos taken during EmpowerMT programs for publicity, promotional, and/or educational purposes (including but not limited to publications, presentations, newspapers, the internet, and other media sources). I do this with full knowledge and consent and waive all claims for compensation for use, or for damages.
