Liability Waiver

WAIVER OF LIABILITY, RECOGNITION AND ASSUMPTION OF RISK AGREEMENT

I acknowledge that I am entering property owned by Pine Knoll, L.L.C. I understand that activity on the property involves physical exertion and outdoor related risks. Accordingly, I hereby waive, release, discharge and covenant not to sue Land Trust for Louisiana and/or Pine Knoll, L.L.C, or any of its agents, servants, or officers from any and all liability, claims, demands, actions, and causes of actions whatsoever arising out of or in any way related to any loss, damage, or injury, including death, that may be sustained by myself, whether caused by the negligence of the releasees or otherwise while on the premises.

I also give my permission for any emergency medical care or treatment by physician, surgeon, hospital or medical care facility that may be required, including transportation, and I accept responsibility for that cost.

I also give my permission for the Land Trust for Louisiana to use photographs and/or video taken of me on this date in publications, news releases, online, and in other communications related to the mission of the organization and understand these images will not be used for commercial purposes.
