Park Condition Assessment

With over 70 city parks to help volunteers steward, Friends of Grand Rapids Parks relies on community members like you to help us keep our parks beautiful!

Please use this tool to survey and assess the condition of your neighborhood park. As you walk through the park, please focus on the following factors: cleanliness, safety, plant/tree conditions, park amenities conditions, and future needs. Feel free to take photos as you walk through and attach them when you reach the end of the form.
