Agreement and Release from Liability

I acknowledge that I have voluntarily applied to the Annual Summer Arts and Music Festival to participate in vending at the Mateel Community Center, 59 Rusk Lane, Redway Ca, 95560. I am aware that hazards exist and I am voluntarily participating in these activities with knowledge of the danger involved. I hereby agree to accept any and all risks of injury or death, or theft, and verify this statement by signing this agreement. As consideration for being permitted by the Mateel Community Center, Inc. to participate in these activities and use their facilities, I hereby agree that I, my assignees, heirs, distributees, guardians, and legal representatives will not make a claim against, sue, or attach the property of The Mateel Community Center, Inc. or any of its affiliated organizations for injury or damage resulting from the negligence or other acts, howsoever caused or any of its affiliated organizations as a result of my participation in the Summer Arts and Music festival. I hereby release The Mateel Community Center, Inc. and any of its affiliated organizations from all actions, claims or demands that I, my assignees, heirs, distributees, guardians, and legal representatives now have or may hereafter have for injury or damage resulting from my participation in The Summer Arts and Music Festival.
I have carefully read this agreement and fully understand its contents. I am aware this is a release of liability and a contract between The Mateel Community Center, Inc. and/or its affiliated organizations and by checking the box I am signing it of my own free will.
