By engaging in activities with Greater Lovell Land Trust (GLLT) staff and other volunteers:
1. I acknowledge that I have voluntarily agreed to participation in a GLLT activity (as indicated on this form), and in consideration of being permitted to participate in such activity, do voluntarily execute this Release on behalf of myself, my heirs, my personal representative, and my estate. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, that may be sustained by me, or any loss or damage or property owned by me, in connection with the activity.
2. By signing this document, I reocognize that there are risks inherent in the activity in which I am participating, and that I will not hold GLLT responsible for any injury or accident.
3. I hearby do authorize the leader of the activity to secure such medical advice and services as may be deemed necessary for the health and safety of myself, and I agree to accept full finanical responsibility.
4. I accept and assume full responsibility for all harm and injury, of every nature, which may occur to me or which I may suffer, and all damages or loss of any personal property or property issued to me by GLLT, and will defend, indemnify, and hold harmless GLLT and its employees, directors and officers from and against any and all claims, demands, actions, or causes of action, on account of damage to personal property, or of my personal injury, or death, which may occur or result directly or indirectlly from my participation in the activity. It is my express intent that this Release shall bind the members of my family, spouse, and my heirs, assigns and personal representative, and shall be deemed as a Release, Waiver, Discharge and Covenant not to sue.
5. I agree to abide by the rules and regulations imposed on participants by GLLT and its employess.
By checking the box below and submitting this form, I the applicant, have read, understood, and agreed to the above.
In addition, by checking the box, I do hearby remise, release, and forever discharge the Greater Lovell Land Trust, its employees, and volunteers from any and all manner of actions, causes of actions, suits, proceedings, debts, dues, contacts, judgements, claims, and demands whatsoever in laws or equity, which against have or which my heirs, executors, or administrators hereafter can, shall, or may have for or by reason of my participation in the activity as a volunteer (updated 5/24/2023).
