2025 Salute Vets with a Smile Volunteer Pledge Form

How does the program work?

The New York State Dental Foundation requires Veterans to request care via an online form, where they will supply proof of Veteran status, share details about their financial need, and explain their dental care need. Our Community Dental Health Coordinator will establish contact with the Veteran to gather more details, offer guidance and manage expectations with respect to potential wait times for dental care. We then work to match the Veteran with a volunteer dentist in their area who may be able to help.

Will I be paid for the work?

If the Veteran has insurance, it can certainly be billed, but we work to make sure that the Veterans are not paying anything out of pocket.

What about lab work?

We cannot guarantee that we will be able to find labs willing to provide free services, but if you have a lab you like to work with and can provide us with their contact info, we can reach out to see if they are willing to help! We can recognize them as a Dental Hero as well.

How will I be recognized?

As a small token of our appreciation, all of the volunteer dentists involved with the program are recognized as Dental Heroes on our website and social media pages. You will be prompted to tells us how you would like your name listed on the form below. If you do not want your name listed, simply write "anonymous" in the form.
