Children Faith Formation - Sacrament Preparation 2018-2019
Please Read the Following Carefully

Students must be in Second Grade or older to be eligible for the sacraments of First Reconciliation and First Eucharist during the 2018-2019 academic year.

In addition:

1. Students must be in the 2nd continuous year of Faith Formation at St. Michael OR in the 2nd continuous year of religion classes in a Catholic school.

2. Parents must provide a copy of the student’s Catholic Baptismal certificate (OR Baptismal date if at St. Michael) as an attachment to the registration confirmation email.

        Parents must provide a copy of the Catholic Profession of Faith certificate for students baptized outside the Roman Catholic Church.

3. A parent must attend the First Reconciliation Information Session in the Fall AND the First Eucharist Information Session in the Spring.

4. A parent and student must attend all required on-site retreats and rehearsals for First Reconciliation and First Eucharist. Dates to be announced.

5. Student must complete First Reconciliation and First Eucharist texts and worksheets prior to receiving the Sacraments.  

If you would like to enroll your student in one of the St. Michael CFF programs, please submit a Sunday Faith Formation Classes/Home School Faith Formation Program Registration Form in addition to this Sacrament Preparation Registration form.

Parent Contract

Since parents are their child’s primary catechists, we invite you to be involved in the Sacrament Preparation program in a special way.

Please read the following expectations for parents with students in the Sacrament Preparation program:

1. Attend one parent meeting for First Reconciliation in the Fall, and one parent meeting for First Eucharist in the Spring. Only one parent is required to attend. Dates to be announced. Nursery will be provided.

2. Teach your student the content of the First Reconciliation and First Eucharist student books, and have your student complete the review worksheets, according to the schedule of lessons distributed at each parent meeting.

3. Attend a First Reconciliation retreat with your student in late November or early December, and a First Eucharist retreat with your student in late February or early March. Only one parent is required to attend. Each retreat is no longer than two hours. Nursery will be provided.

4. Bring your student to the First Reconciliation and First Eucharist rehearsals.

 

I understand my role in my student’s sacrament preparation, and I agree that our family will fulfill the requirements listed above. I understand that not fulfilling one or more of the requirements will result in delaying my student’s celebration of First Reconciliation and/or First Eucharist.
