In consideration of the opportunity afforded me to assist on a voluntary basis with Modcon Living, a project in which the homes of disadvantaged persons will be repaired by volunteers, and in light of the aims and purposes of community service provided by Modcon Living in organizing this project, I hereby waive any right or cause of action arising as a result of my participation in said project from which any liability may or could accrue against Modcon Living, its officers and directors, collectively or individually. Without limiting the generality of foregoing, I agree that this waiver shall include any rights or causes of action resulting from personal injury to me or damage to my property sustained in connection to my activities for Modcon Living.
Modcon Living requests your permission to take or have taken, still and moving photographs and films, including television pictures. By signing this waiver, you consent and authorize Modcon Living, its advertising agencies, news media and any other persons interested in Modcon Living and its works, to use and reproduce the photographs, films and pictures and to circulate and publicize the same by all means including without limiting the generality of the foregoing, newspapers, television media, brochures pamphlets, instructional materials, books and clinical material. No inducements or promises have been made to secure your signature to this release other than the intention of Modcon Living to use such photographs, films and pictures for the primary purpose of promoting and aiding its program and its works
Covid-19 agreement:
- Not to participate in the Project if I have had a body temperature of 100.4 degrees or higher during the previous 14 days or have suffered any symptoms of the COVID-19 virus, the flu, or the common cold;
- To comply with all project management, sanitation and cleanliness and “social distancing” rules, policies and guidelines that are applicable to the Project, including with respect to hand washing, wearing a protective face mask and gloves, and using my best efforts to maintain an appropriate distance from others; and
- That I (a) am participating in the Project at my own risk and may be exposed to persons carrying or who exhibit symptoms of the COVID-19 virus, (b) have voluntarily assumed such risk and (c) have waived and released any and all claims related to the foregoing under the Assumption of Risks section.
