You are helping the Community Care Clinic to continue to provide quality primary care!

Individual patient fees are below. If you live in a household with multiple members, please call the clinic to determine your fee.

Amount

$45 - For individual annual income less than $14,580

$80 - For individual annual income between $14,581 and $21,870

$115 - For individual annual income between $21,871 and $29,160

$150 - For individual annual income between $29,161 and $36,450

$185 - For individual annual income between $36,451 and $43,740

Donation Schedule

Patient Administrative Fees can be pain in-person.

If you are unsure what your Patient Fee is or have questions, please call the clinic at 252-261-3041. For the staff at the clinic to determine your fee they will need your yearly income and the number of individuals in your household.

To pay by CREDIT CARD click the DONATE ONCE button below then click the PayPal Check out BUTTON. On the next screen, scroll down and click on "Pay with Debit or Credit Card". Enter your credit card information and scroll down to "Continue as Guest".
