Background Check, Liability and Media Release, Code of Conduct

You will be asked to fill out a background check, sign a Liability and Media Release, and agree to a Code of Conduct when you attend your first orientation. We conduct background checks on all dedicated volunteers due to our programming with school children and vulnerable populations. If you would like to review our forms, they are listed below. You do not need to fill out the forms at this time, we will provide them at your first orientation. If you are under 18 years of age, you will need a parent or guardian to sign off on this form. If you have any questions or concerns, please contact us.
