RESOURCE GUIDE APPLICATION 2024

Thank you for joining the growing number of providers in southwest Montana who are expanding their ability to work with families in the perinatal period. The Roots Resource Guide: Connection and Support from Pregnancy to Parenthood highlights those providers who have received specific training in Perinatal Mood and Anxiety Disorders (PMADs) as well as those who have experience working with families in this unique developmental stage. It will serve as a safety net for families seeking care and providers seeking referral options.

*This Guide will not serve as a recommendation or endorsement of specific providers, but will simply serve as an informational listing for those seeking support.

Review eligibility requirements & fees, completely fill out the application, and submit payment below.

Once submitted, your application will be reviewed by the Resource Advisory Committee and you will be contacted if they have any additional questions.

‚ÄčIf your application is approved, you will be notified and your listing will be added to the guide and printed in January 2024. If your application is not approved, you will notified and will be refunded your application fee.

The deadline to sumbit your application is October 27.

Early bird registration (10% discount) is October 16.

2. Before moving forward does your application meet the eligibility requirements to be listed in this Guide?

Mental Health Providers -- minimum of 12 CEUs in PMADs, 2 years working with this population, current license to practice, & yearly commitment to obtain CEUs in PMADs.

All other perinatal professionals -- minimum of 5 CEUs in PMADs, 2 years working with this population, up-to-date and licensed in your field, yearly commitment to obtain CEUs in PMADs

The main goal of this guide is to connect parents to appropriate resources in the easiest way possible. When filling out your Listing Information, please consider those who will be seeking your services.

*Consider these questions as you are writing -- how do you address perinatal mental health in your office. Do you offer in-home services? Do you have a protocol for screening, referral, and followup? Do you have a Behavioral Health Professional on staff? Do you or your staff have training to work with/understand the greater challenges faced by BIPOC and LGBTQIA+ families? Do you work with partners/dads?

If you are submitting your application to reflect more than one listing in SEPARATE categories, please check all that apply. Each separate listing will be charged. For example, if your business/practice has both Acupuncturists and Midwives to be listed in the Guide, you will need to check both Acupuncturists and Midwives in this field and will be prompted below to specify each listing.
