Thank you for registering for Cleveland Eye Bank Foundation's 4th Annual Golf Tournament, "Keep Your Eye on the Ball," on Monday, June 17th, at Pine Hills Golf Club, 433 West 130th Street, Hinckley, OH 44233.

One registration form is required of each golfer or team and payment is required upon registration. If you plan to pay for more than one golfer in your foursome, payment options are below. If you pay as a solo golfer, please communicate to your team members that they also need to register and pay. Finally, please note that if you are playing as a single golfer, twosome, or threesome, we may fill your group to round out foursomes.

Payment

Each golfer is responsible to pay $125 and will automatically be included in the tournament's skins game.

Payment is required upon registration. If you do not wish to pay via credit card, please make checks payable to Cleveland Eye Bank Foundation and mailed to 18720 Chagrin Boulevard, Cleveland, OH 44122.

If you would like to include event sponsorship or donation with your registration, please select the "Other" amount and enter your golfer payment PLUS your sponsorship or donation amount. Payment for your sponsorship is not required to submit this form.
