River Clean Up Waiver - Youth

This Release and Waiver of Liability (Release) executed on this date by the legal guardian of participant(s) named below releases the Little Miami Watershed Network (LMWN), a nonprofit corporation organized and existing under the laws of the State of Ohio and each of its directors, officers, employees, volunteers and agents.

1. Waiver and Release: I, the legal guardian of the youth participant(s) on behalf of the youth, release and forever discharge and hold harmless LMWN and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the services the youth receive from to I, on behalf of the youth listed below, understand and acknowledge this Release discharges LMWN from any liability or claim I, on behalf of the youth,  may have against LMWN with respect to bodily injury, personal injury, illness, death, or property damage that may result during my participation in the Little Miami River Clean Up.
2. Insurance: Further I, on behalf of the youth, understand LMWN does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health or disability benefits or insurance of any nature in the event of my injury, illness, death or damage to my property. I, on behalf of the youth, expressly waive any such claim for compensation or liability on the part of LMWN beyond what may be offered freely by LMWN in the event of such injury or medical expenses incurred by

Medical Treatment: I, on behalf of the youth, hereby Release and forever discharge LMWN from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my participation in the Little Miami River Clean Up.
4. Assumption of Risk: I, on behalf of my child or ward, understand that they are participating in the Little Miami River Clean Up in response to a request by LMWN. They receive their initial instruction from LMWN personnel or volunteers and fulfill their task under those instructions. In addition, I, on behalf of my child or ward, understand that the event they participate in may include activities that may be potentially hazardous to them including, but not limited to walking on grass covered grounds which may be uneven or wet. These activities could result in injuries from scrapes and cuts to sprained ankles or broken bones, etc.
5. Photographic Release: I, on behalf of the youth, give to LMWN all rights, title, and interests in any and all photographs or recorded images of me made by LMWN in connection my participation.
6. Other: I agree this Release is intended to be as broad and inclusive as permitted by the laws of the State of Ohio and this Release shall be governed and interpreted by Ohio I, on behalf of the youth, agree that if any clause or provision of this Release is deemed invalid, the enforceability of the remaining provisions of this Release shall not be    affected.

By checking the boxes below, I, as legal guardian on behalf of the youth, express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily.
