Electronic receipt of this completed registration form constitutes agreement and understanding by the child's parent and/or guardian, with the following:

My child has permission to attend the Missoula Children's Theater 2019 production of Gulliver's Travels from Monday, July 8 to Friday, July 12, from 9:00am to 3:30pm. The camp is held at Shorewood High School in the Performing Arts Building, 17300 Fremont Ave. N. Shoreline, WA 98133.

The performance is at 1pm on Friday. The cast party is from 2:30-3:45 directly after the performance.

Aftercare is available to those who register separately on this form.

Arrive for auditions by 9am on Monday morning. Auditions are for role placement and begin promptly at 9:15am. No late entries. No adults allowed in the audition room. 

Campers 12 years old and older must give written consent to leave the camp location alone, or with someone other than a parent. All campers must sign out each day with the Shoreline-Lake Forest Park Arts Council staff member on site.

 Every student who registers will receive one ticket to the show. You will also have the opportunity to purchase additional tickets to the show for $10 each (children 5 and under are free). You can pay by check (preferable), cash or credit card. Every student in the show will receive a digital download code for a professional video of the performance about 2-3 weeks after the show.

I consent to the use of any photos or videos taken during the production residency for marketing purposes.  I understand that a video of the performance will be made available to the cast but will not be available to the general public except for marketing purposes.

Receipt of this registration constitutes an agreement that the Shoreline-Lake Forest Park Arts Council, the Shoreline School District, the City of Shoreline and the City of Lake Forest Park, Missoula Children's Theater, 4Culture and the Washington State Arts Commission will be held harmless against any/all injury, loss, or damages. All reasonable precautions will be taken to prevent any injury, loss or damage.
