COFI · COFI Center Workshop Registration - Check/Money Order or Contract

Please complete a registration form for each participant and submit payment in order to register for COFI Center workshops in 2023-2024.

Please note that this form is for check/money order payments, or if you already have a contractual agreement with COFI. If you would like to register for COFI Center workshops but pay by credit card online, please fill out this form.

If you prefer to complete a paper registration and mail it in, you may download the form here.

COVID Policy: Proof of vaccination is not required. Masks are optional.

Please note: Registration is non refundable. We can transfer the registration to another person or credit your registration towards a future training.

Tuesday, 3/19–Friday 3/22/24, 9 a.m.-4 p.m. every day except on Friday, training ends at noon. Lunch will be provided every day except Friday.
