2018 Holiday Basket Event Registration

DEADLINE TO PRE-REGISTER: WEDNESDAY, NOVEMBER 21st

Date: Saturday, December 15, 2018
Time: 11:30-4pm (specific check-in time will be assigned)
Location: Kellogg Middle School (16045 25th Ave NE)

Eligibility Requirements for Holiday Basket Recipients:
Must have a student in the Shoreline School District for the 2018-2019 school year OR a child 0-18 years of age living within the cities of Shoreline or Lake Forest Park.

TO REGISTER FOR THE EVENT:
Submit the following form online (preferred) OR with the family advocate at your child's school.

CHECK-IN TIME WILL BE ASSIGNED via a letter sent by US Mail (and email, if provided). Your letter with an assigned check-in time should arrive the week of December 3rd. The letter will be in English.  Check the box below to receive the family letter in Spanish. 

We ask that you leave your students/children at home to protect their privacy as well as the privacy of other students.
