Participant Voluntary Feedback

This year, we are working with a team from the Data Innovation Project at the University of Southern Maine to help us assess the quality, impact and effectiveness of our programs. We will be asking youth to fill out feedback forms after activities and conducting an annual survey. These feedback opportunities will give youth a chance to share their experiences with the program(s) they are a part of and will help us to learn how OSI might have impacted the way they think about themselves, outdoor sport and community. No names or personally identifiable information are recorded or attached to the survey forms or data. Participation is voluntary. All data collected will be kept anonymous and shared in aggregate with staff, funders and supporters of OSI.  By signing below, you understand and give permission for your child to fill out feedback forms and/or assessment tools. If you have any questions about this process, please contact Mike Smith (mike@outdoorsi.org / 207.227.0250).‚Äč
