Youth Acknowledgement of Risk

We require that every youth who participates in a Project Watershed field trip or volunteer event read this Acknowledgement of Risk form with their legal guardian. We do our utmost to make our activities engaging, safe and enjoyable, but as a non profit organization, we have legal and insurance needs that must be clearly written and fully understood by those who choose to participate in our activities.

The Activities
Activities will include walking in natural habitats with uneven and often wet/mucky ground (e.g. forest, beach, mudflats), playing tag, doing a science activity involving salt, water and dye, catching and examining fish with a net, planting native plants, weeding/removing invasive species, building a replica fish trap, picking up garbage, and squirting water onto a tarp to demonstrate how a watershed functions.
 
Risks
Known risks of participation in these activities include tripping/falling, getting wet/dirty, dehydrating, or overheating.
 
Acknowledgement of Risk
In consideration of the Participant named below being given the opportunity to participate in a Project Watershed field trip, I acknowledge and agree that I understand the nature of these Activities and that they are able to participate in such Activities. I further acknowledge that The Activities will be conducted at public parks and facilities open to the public during The Activities and upon which the hazards of traveling are to be expected.
 
I AM FULLY AWARE that The Activities entail risk and I understand the description of these inherent risks is not complete and that other unknown or unanticipated inherent risks may result in injury or death. There may be other risks or social and economic losses either not known to me or not readily foreseeable at this time, and I fully accept and I assume all such risks and all responsibility for injuries, losses, cost and damages as a result of participation in The Activities attendant thereto, and thus release, and agree to indemnify and save and hold harmless Project Watershed and its directors, employees, volunteers, administrators, and members from any and all liability, responsibility and/or property damage, losses, damages, claims, demands or causes of action against them arising from or attributable to participation in this event. I also release, waive, discharge, and relinquish anybody associated with Project Watershed including sponsors, leasers of premises on which the Activity took place, involved public entities, and their respective owners, officers, employees, agents, representatives, successors and assigns from any and all liability, responsibility and/or property damage sustained during participation in this event.

The Participant may be photographed for use on Project Watershed’s website, social media channels, events, and news. By choosing yes below, I acknowledge and consent to Project Watershed using The Participant’s image and/or voice and of images of any works the participant may create as part of this field trip (collectively, “Materials”), in Project Watershed’s digital and print promotional, fundraising, educational, and other communications. Project Watershed will not include names of photographed participants in any Materials. Project Watershed may use the Materials without obtaining my approval or paying me for such use. I grant Project Watershed all copyrights in and waive any legal claims relating to the Materials, including those relating to copyright, rights of publicity or privacy, or defamation, or arising from any distortion, blurring, or alteration that may occur in the making, editing, or use of the Materials.
