All participants shall carefully read and sign this form. For participants under 18 years of age (‘minor’ or ‘child’), parents and or legal guardians must also sign.
I freely consent to participate in a project sponsored by Teton Creek Corridor Partners (TVTAP, TRLT, VARD, FTR), nonprofit 501C(3) charitable organization. I agree to listen to, and follow, all safety instructions and procedures presented in conjunction with this project. I further agree to use my best judgment based upon my physical and mental abilities at all times, and to immediately terminate participation in this project if activities become too strenuous, difficult or hazardous for me. I am aware that the project may include inherent risks or hazardous activities in a natural environment, and may take place on a location or under conditions that may be dangerous to me. I am aware that participation in this project poses certain dangers, including, but not limited to, the forces of nature, the hazards of traveling and hiking in wilderness and often steep mountainous terrain, using hand or motorized construction tools (and of working in the proximity of such tools when used by others), and injury or illness in remote places without medical care. I understand that the Teton Creek Corridor partner organizations cannot eliminate the forces of nature, any of these inherent risks, or assure the safety of participants in this physically strenuous project. I freely accept full personal responsibility for all risks, illness, injuries, loss, damage, disability, or death arising from, or relating to, this project.
In consideration for permitting me to participate in this project, I, for my family, my estate, and myself, hereby waive any right of recovery and claims of liability against Teton Creek Corridor Partners, its officers, employees and agents, including claims for bodily injury, including death, personal injury and/or damage to property, and release the Teton Creek Corridor Partners its officers, employees and agents and release from such claims and any claims made by others for personal injury or property damage allegedly caused by me. Further, I will hold the Teton Creek Corridor Partners harmless from any loss or damages resulting from the foregoing waiver and release. This Release and Indemnity Agreement is a contract not a mere recital and thus it shall remain in effect for all projects sponsored by the Teton Creek Corridor Partners.
The undersigned gives his/her permission to be photographed/filmed and have his/her image used by the Teton Creek Corridor Partners without royalty or compensation. The partners may recognize volunteers by listing their names in future publications.
PARTICIPANT AND PARENTS/GUARDIANS OF A MINOR PARTICIPANT AGREE: I HAVE CAREFULLY READ, UNDERSTAND AND VOLUNTARILY SIGN THIS DOCUMENT AND ACKNOWLEDGE THAT IT SHALL BE EFFECTIVE AND BINDING UPON ME, MY MINOR CHILDREN AND OTHER FAMILY MEMBERS, AND MY HEIRS, EXECUTORS, REPRESENTATIVES AND ESTATE. Parents/Guardians must sign below, both in their capacity as a participant, and as Parents/Guardians of any minor child identified below.
