The Provider Collaborative & Resource Guide Application (2025)

Welcome to The Provider Collaborative

Thank you for joining the expanding network of providers in southwest Montana dedicated to supporting families during the perinatal period–from preconception to parenthood.

We’re excited to introduce The Provider Collaborative—a NEW, membership-based initiative offering partnership plans with exclusive benefits designed to strengthen connections among providers and amplify community impact.

ALL Partner Plans include a listing in 2025 Roots Perinatal Resource Guide & Online Directory.

The Perinatal Resource Guide for growing your family will feature providers trained in Perinatal Mood and Anxiety Disorders (PMADs) and those with experience working with families during this pivotal stage. Serving as a comprehensive directory, this guide will be a vital resource for families seeking care and for providers looking to establish referral networks.

*This guide is not intended as a recommendation or endorsement of specific providers. Instead, it serves as an informational listing for those seeking support.

To register for The Provider Collaborative, and be included in the Guide:

  1. Review the eligibility requirements and partner plans
  2. Complete the application
  3. Submit your payment using the link below

Your registration application will be reviewed by The Provider Collaborative Advisory Committee, and you will be contacted if further information is needed.

If approved, your listing will be added to the guide, which will be printed in January 2025. In the event your registration is not approved, you will be notified, and your registration fee will be refunded.

Eligibility Requirements 

We highly value providers with experience working with perinatal clients. If you don’t meet all the requirements but believe your experience qualifies you for inclusion, we encourage you to apply. We will review your registration application thoroughly and reach out if we have any questions. Mental Health Providers:

    • CEUs: Minimum of 12 CEUs in PMADs (Perinatal Mood and Anxiety Disorders).*
    • Experience: At least 2 years of experience working with this population.
    • Licensing: Must hold a current license to practice.
    • Commitment: Yearly commitment to obtain additional CEUs in PMADs.*

All Other Perinatal Professionals:

    • CEUs: Minimum of 5 CEUs in PMADs.*
    • Experience: At least 2 years of experience working with this population.
    • Licensing: Must be up-to-date and licensed in your field.
    • Commitment: Yearly commitment to obtain additional CEUs in PMADs.*

*If you do not meet the PMADs CEU requirements, but offer a valuable service, we encourage you to apply. 

Partner Plans

There are three Partner Plans available, tailored to the size of your practice.

  1. Support Plan: This option includes the essential service providers have always received—a listing in the resource guide and online directory—at the same price as before.
  2. Connect Plan: In addition to the listing, this plan offers access to learning opportunities, social networking events, and information-sharing benefits.
  3. Grow Plan: This all-inclusive option includes everything in the Connect Plan, plus up to 16 hours of reserved space at Roots each year for your programming needs, along with the opportunity to apply for program partnerships with Roots.

Pricing and benefit details are available in the membership guide. 

You’ll have the chance to submit suggestions and sign up to host/lead various benefit opportunities in this application. The finalized calendar will be shared by the start of the new year!

Key Deadlines:

  • September 30: Early bird registration deadline (10% discount).
  • November 1: Final deadline to submit your application.
  • November 27-30: Review resource guide listing for print over Thanksgiving Break
If you were listed in 2024 and have absolutely no changes to your credentials, bio, contact information, logo, or any other details, please complete sections 1, 2, and 5 to release permission for expediting your application process.

3. RESOURCE GUIDE LISTING INFORMATION

The primary goal of the Perinatal Resource Guide is to connect parents with the appropriate resources as easily as possible. When completing your Listing Information, please keep in mind the needs of those seeking your services.

As you write, consider these questions to guide your description:

  • How do you address perinatal mental health in your practice?
  • Do you offer in-home services?
  • Do you have protocols for screening, referral, and follow-up?
  • Is there a Behavioral Health Professional on staff?
  • Do you or your staff have training to support the unique challenges faced by BIPOC and LGBTQIA+ families?
  • Do you work with partners/dads?

For example, if your business or practice includes both Acupuncturists and Midwives, you will need to select both categories and specify each listing below.

Grow Partners are invited to apply to partner with Roots to offer supportive programs that support our mission. Are you interested in partnering with Roots?
