Required Documentation

In addition to completing this application, please share copies of the required documentation listed on our website regarding your proof of income, financial hardship, and rental costs. Drop off your documentation at a secure mailbox located at 4215 W. Washington, upload them to this form, or email them to organizer@gpcommunitycouncil.org.

The following documents are required, at minimum. You may be asked to provide additional documentation.

Identification and Proof of Residency: 

Government Issued ID or Drivers’ License with current address

Household Income and Financial Hardship (after March 1):

Unemployment Claim/Notice dated after March 1, OR: Documentation from former/current employer of layoff, furlough or reduction in hours (paystubs), OR: Proof of self- employment and of reduction in income; Proof of self-employment (e.g. EIN/TIN number or business license.); Paystubs or bank statements showing decreased income after March 1

Rental Documentation:

Lease document OR Landlord Affidavit (letter from landlord validating rent agreement), AND:

2 Bank statements or canceled checks showing payments to landlord, OR: Last two rent receipts, OR: 5, 10, 15, 30-day late notice (with the amount)
