Host Gym/Box Sign Up Form - 22 WOD

This form is to sign up gyms as OFFICIAL HOST GYMS. Please complete this form if you are a Team Leader and will be signing up individuals at the gym/box the day of the event. Team Leaders are responsible for collecting payment and donating funds to the Honor and Courage Foundation within 14 days of the close of the event. Once signed up you will receive further instructions via email within 72 hours. Completing this form does not register you as an individual.
