I am aware of the current risks associated with the COVID-19 pandemic and that the situation is constantly changing. I know it is my responsibility to stay up to date on the latest risks and that I am required to monitor my camper for signs and symptoms related to COVID-19 during the 2 weeks prior to camp, the 5 days my child is attending camp and two weeks after, as outlined in the Columbia Springs Summer Day Camp Health and Safety Policy linked below. If anyone in our household becomes ill with the COVID-19 virus in the 2 weeks preceding, or during camp, my child will not attend this camp and I will e-mail or call Columbia Springs to cancel. If my camper was possibly exposed to someone who has COVID-19 before or during camp my camper will not attend without testing negative for COVID-19 within 24hrs before the start of camp. I understand that I am eligible for a refund of program fees minus credit card processing charges. I agree that any photographs or digital images taken by Columbia Springs personnel of my child as a program participant, and copies of artwork made by my child while a Columbia Springs program participant, shall be the property of Columbia Springs, and may be used by Columbia Springs, at its discretion, for any publicity, education, marketing and/or advertising purposes.
