Please use this form to make your online payment and secure your booking request. Payment by check can be made out to Seattle JazzED and mailed to Seattle JazzED, 380 Boren Ave N, Seattle, WA 98109. Please note that 50% payment is due in full upon receipt of the invoice and we will only move forward with your booking after payment has been received. All payments made to Seattle JazzED are refundable until two weeks prior to your event. Events and bookings canceled after this period, will be subject to a 50% penalty of the total amount due. This penalty will be used to compensate our musicians for holding this date.
