Gift of Life Sponsorship Purchase Form

Hello! Thank you for choosing to honor a child's second chance at life by sponsoring our Gift of Life 5K Run/Walk event on October 5, 2024.

This year’s event will be held on October 5th, 2024, and our focus is to raise contributions and awareness towards honoring a child's second chance at life.

Thank you for considering participating in the 3rd annual Gift of Life 5K Run/Walk. As before, this race will benefit the transplant patients and families of Transplants for Children (TFC).

Presenting Sponsor - $5,000 (1 available)

All material will read "Name of Sponsor Presents Gift of Life 5K Run/Walk 2024" with logo
Recognition in all media as the presenting sposnor
Large baner with company logo at start/finish line and water stops (must provide banner/signage)
Complimentary entries for five (5) five-person teams (25 registrations)

Hero Sponsor - $2,500

Small yard sign with company logo at registration (must provide banner/signage)
Ability to sell company products in health & wellness fair
Complimentary entries for three (3) five-person teams (15 registrations)

Partner Sponsor - $1,500

Area in health & wellness fair (please provide table/chairs)
Ability to offer samples, coupons, and company infographics (no selling items)
Ability to contribute to the "schwag" bag (up to 300 pieces needed by September 24)
Ability to contribute door prizes to be raffled off
