Community Farm Store Volunteer Form

We recently moved our farmstand to a beautiful retail space in Prairie Crossing Station Square, located at 970 Harris Rd, Unit 1B in Grayslake. Our new location allows for greater access as folks can get there by car, bike, foot, or train. We offer food and retail products from our farm, other Midwestern farms, and businesses committed to sustainability. In addition to the shop, our new location features a community room that is available for small groups rentals.

We are looking for volunteers to help staff the Farm Store. We are open Wednesday through Sunday from 9:00am-5:00pm. We prefer individuals who can contribute regular 2-4 hour shifts. All volunteers receive 5% off in the store and those who contribute 8 hours monthly receive access to the community room free of charge.

We welcome families to volunteer. Please note that volunteers 15 years old and under must be accompanied and supervised by an adult at all times while on Liberty Prairie premises. 

I release and hold harmless the Liberty Prairie, its officials, agents, volunteers, employees, and their successors (collectively, “the organization and its successors”) from any and all claims, costs, suits, actions, judgments, or expenses for any damage, loss, or injury to me or to my property which may arise from this volunteer event. I further release and hold harmless the organization and its successors from any injury caused by any first aid, treatment, or service administered to an injured volunteer.

I acknowledge that I am fully aware of any and all risks posed by these volunteer activities and that I have no medical condition that prevents me from engaging in them. 

I agree to follow all safety rules and instructions that are designed to protect the participants’ safety. I understand that if I or any minor child in my care engages in behavior deemed by Liberty Prairie to be unsafe to myself or others, I/we will be asked to leave.

I give permission to be photographed or video recorded by Liberty Prairie, project partners, or the media for use in printed materials, through the internet or through other media outlets. Such photos remain the property of Liberty Prairie. I understand that if I do not wish to be photographed/videotaped, a written objection must be filed with Liberty Prairie.

Liberty Prairie works with school groups and some districts require background checks. Student programs will expand such that some volunteers will be student interns in the store. With this in mind, Farm Store volunteers are required to submit fingerprints and deliver a successful background check. In addition, all volunteers will be required to confirm their emergency contact information and sign a physical copy of this release. For volunteers under the age of 18, a parent or legal guardian will be required to sign as well. 

By agreeing to these terms, I acknowledge that I have read and understand this waiver and release.
