Proceeds from the sale of my artwork will be 100% donated to ShoreLake Arts. Artists who donate their commission will be thanked by name on the SLA website, in our electronic newsletter, and on a special social media post.

Electronic receipt of this completed registration form constitutes agreement with all the requirements, guidelines and instructions detailed in the prospectus for participation and entry in the 6X6NW art exhibition and fundraising event. It is the responsibility of the artist to provide their own art insurance if desired. Receipt of this registration constitutes agreement that ShoreLake Arts is in no way responsible for loss or damage to any artwork. All reasonable precautions will be taken to prevent any loss or damage. Consent to reproduce any artwork for promotional purposes is assumed to be granted unless specifically reserved in writing to ShoreLake Arts. Artist agrees to hold harmless ShoreLake Arts, the Cities of Shoreline and Lake Forest Park, Shoreline Community College, Shoreline School District, their employees, and invitees, including the general public against all injury, loss, or liability occasioned as a result of participating in this event.
