Submission of this request form;
A copy of your Safe Church Certificate (training completed within the past 5 years) uploaded via this request form;
A letter of recommendation from your clergy mentor sent to office@cnyepiscopal.org;
A letter of recommendation from a/the leader of the Lay Preacher program sent to office@cnyepiscopal.org.
