I, the undersigned, as the legal guardian of the child named below, do hereby give my full consent and approval for my child to participate in the Garden Rangers program.
I understand that there are some inherent risks in participating in activities that are outside, and I am willing to assume these risks on behalf of my child. On behalf of my child and myself, I voluntarily elect to accept and solely assume all risks of injury incurred or suffered by my child while participating in this program. In addition to giving my full consent for my child’s participation, I do hereby waive, release, discharge, and agree not to sue the Garden Rangers program, Plenty!, or any of its leaders in the event of any damages or injuries when participating in this program.
I hereby certify that my child is fully capable of participating in the Garden Rangers program.
I acknowledge that I have read (or have had read to me) the above waiver and release of liability agreement and I agree to abide by them.
By signing this agreement, I agree to the provision of medical care in case of an emergency.
