Please use this form if a one certified Delegate will be replacing another certified Delegate.

This form may be used for:

Certified Alternate Delegate replacing a certified Lay Delegate (most common)
Newly certified Lay Delegate replacing a previously certified Lay Delegate (in the event that previously certified Alternate Delegates were not able to serve)
Newly certified Alternate Delegate replacing a previously certified Alternate Delegate (in the event that previously certified Alternate Delegates were not able to serve)

Please note that if the replacing Delegate was not on the parish's original Lay Delegate Certificate, they must be certified first. Please use the Lay Delegate Certificate form to certify the new, replacing delegate, then complete this Delegate Change Form.
